Report of the Apollo 204 Review Board (Apollo 1 fire)
Report · written 5 April 1967 · released 5 April 1967
Why this matters
The investigation of the January 1967 launch-pad fire that killed Gus Grissom, Ed White and Roger Chaffee in a pure-oxygen cabin full of flammable material. It names the design and quality-control failures, which led to a redesigned hatch and spacecraft before any Apollo crew flew. NASA's history site hosts the full report with appendices.
